  • Patient Name, Address, and Phone Number
  • Subscriber Name, Address, and Phone Number (if different from patient)
  • Healthcare Provider Name, Address, and Phone Number
  • Name and Phone Number of Insurer(s)/Payer(s)
  • Policy ID Number(s)
  • Employer and Group ID Number
  • Name, Dose, and Strength of GSK Drug
  • Copy of Insurance Card (if applicable)

Some of the information elements listed above will not be applicable in all situations. Patients will be required to sign a written release of medical information and authorization statement in order for GlaxoSmithKline to provide patient-specific assistance.
